For all LCSWs and CSWAs who were born in August, please note that your renewal forms were mailed from the Board office on Wednesday, July 1, 2009. You should have received your renewal information a long time ago. Hopefully, you not only received your renewal form but have already renewed your License or Certificate. If so, disregard the rest of this notice.

Remember, it is your responsibility to timely renew your Certificate or License. To avoid paying a $50 late fee, you’re renewal must be “filed with and accepted by the Board” by August 31, 2009. [OAR 877-020-0020(8)]
The Board suggests you send in the completed renewal form with the required fee within 48 hours of when you receive your renewal notification.
