The renewal application is now also available online (fillable pdf format) and will also be sent by regular mail.  This renewal application can be used by Oregon Certified Public Accountants (CPAs) or Public Accountants (PAs) whose license expires on June 30, 2013.  The online application is generic and does not have personal information pre-populated.  If you would like to use the online form, please use the licensee look-up to find your current carry over hours, ethic CPE requirement and pro-rated hours, as applicable, or refer to the pre-populated form you will receive by mail.

The renewal application can be used regardless of your current status (active or inactive) and regarless of what status you wish to renew as.  The Board will not accept emailed forms.  If you choose to use the online renewal application, please do not also send in the paper application you will receive by regular mail.

List Request Form If you would like to purchase a list of CPAs, PAs, Exam Candidates, Firms or Municipal Auditors, you must download this form and send to the Board office.


Disposition Form You must download and complete this form if you have ever been charged, convicted, pleaded nolo contendere or found guilty of any criminal offense (excluding non-criminal traffic violations).

You must also complete a disposition form if you are currently or have previously been under investigation by any governing or licensing board or have ever had a professional or vocational license denied, suspended, or revoked by this or any state, jurisdiction or foreign country.
