REMEMBER - Sending in the questionnaire is only a first step: Once BOLI receives your questionnaire, you may be contacted to provide further information. When BOLI has determined that there is enough information to proceed, BOLI will prepare a complaint and send that complaint to you for your approval and notarized signature. You will not have officially filed a complaint until BOLI receives that signed and notarized complaint.
Important: If your contact information changes at any time, you MUST notify BOLI of your updated information. If BOLI is unable to contact you, your questionnaire may not be processed, or your complaint may be dismissed.
