Applying for a License
Speech-Language Pathology License
To apply for a speech-language pathologist license, you will need to submit the following:
  • The application form
  • A Check or MO for at least the $200.00 application fee. 
    • Most applicants send in the $275 licensing fee at the same time to save some time. You may send both fees on one check ($475).
    • Make the check payable to “Speech Board”.
  • Official transcripts just for your Master’s degree.
    • You may send these in if they are in a sealed envelope from the university; otherwise they need to come from the university to us.
    • We only need the Master’s degree transcripts, unless you are having the transcripts reviewed by the Board.
  • Do you have your CCCs?
    • If you do, please contact ASHA and have them send verification to us.  (Faxed is fine)
    • If you don’t, you’ll need to send:
      • Copy of your NTE Praxis scores for SLP (score must be 600 or over scare)
      • Was your school accredited by ASHA when you graduated?  If it wasn’t, you’ll need to include:
        • Proof of 275 Clinical Clock Hours
        • Proof of 9 months supervised experience
  • Provide 30 hours of professional development within the last 24 months (see application form for details).
  • These documents do not need to arrive all at once.  We will set up a file for you upon receiving one of the above items and collate the others as they arrive.

Speech-Language Pathology Assistant Certificate
To apply for a speech-language pathologist assistant certificate, you will need to submit the following:
  • application form
  • A Check or MO for at least the $200.00 application fee. 
    • Most applicants send in the $150 licensing fee at the same time to save some time. You may send both fees on one check ($350).
    • Make the check payable to “Speech Board”.
  • Official transcripts that show:
    • 30 semester or 45 quarter hours credit in general coursework, AND
    • 30 semester or 45 quarter hours credit in technical (related to slp/communication) coursework.
    • You may send these in if they are in a sealed envelope from the university; otherwise they need to come from the university to us.
  • SLPA Competency Checklist
    • If you have a Board-approved practicum on your transcripts for at least 100 clock hours, this will substitute.
  • These documents do not need to arrive all at once.  We will set up a file for you upon receiving one of the above items and collate the others as they arrive.

Audiologist License
  • application form
  • A Check or MO for at least the $200.00 application fee. 
    • Most applicants send in the $275 licensing fee at the same time to save some time. You may send both fees on one check ($475).
    • Make the check payable to “Speech Board”.
  • Do you have your AuD from an accredited university?
    • If yes, submit official transcripts for that program
    • If no, do you have your Master's from an ASHA accredited program?
      • Yes:  submit the official transcripts for that degree and evidence of 9 months supervised experience.
      • No:  You'll need to submit official transcripts for the degree, evidence of 275 clinical clock hours, and evidence of 9 months of supervised experience.
