Note:   As of May 1, 2008, we are not accepting new arbitrator applications.   For the best administration of the arbitration program, it is necessary to limit the number of arbiters to a total of 100. That number has been reached.   An announcement will be posted on this site as soon as applications are again being accepted.

CRIMINAL FORMS

We now have two criminal forms available that are fillable PDF forms.

If you choose to use these forms, you must provide the court with an original and two copies of the Petition to Enter Plea of Guilty and an original and one copy of the Appeal Rights.

For the Petition to Enter Plea of Guilty, please follow these steps:

  • The original must be signed by the attorney;
  • On the remaining copies, under box number 10, check the box that says "I certify that this is a true copy of the original."
  • Initial each copy in the signature line (do not sign).
