How agencies can engage with e-government for Forms Factory services
 
  1. Contact E-government to set up an initial discussion session.  Topics will include general information about services available, pricing, how the service works, and general business needs of the agency.
  2. Complete an Interagency Agreement (IAG) with the E-government Program.  This agreement is an "umbrella" agreement that allows all work to be done via signed Statement of Work (.doc)  (SOW).  The actual dollar and service commitments are in the SOW, not the umbrella agreement.  An example Interagency Agreement can be found here (.doc).
  3. Agencies will complete a Forms Factory Service Request (.pdf) for each e-form to be developed and delivered under the agencies' IAG with e-government.  Once Forms Factory receives this form, Saber staff will meet with the agency to further refine requirements and begin work on an SOW.
  4. Forms Factory staff will work with the agency to create a Statement of Work (SOW) for each e-form. The agency and all participants will sign off on the SOW, which will include the finalized Forms Factory Service Request.
  5. The SOW and Forms Factory Service Request will be submitted to E-government. E-government will provide the final cost for the e-forms services requested based on the rate schedule.
  6. The agency will review the final quote and on approval, will send notification to E-government which will initiate the development process.
  7. Saber staff and agency iteratively design and test the form until the agency agrees the form meets the agency business need.
  8. Agency approves the e-form and sends notification of acceptance to E-government.
  9. E-government bills the agency for e-form development.
  10. Agencies post e-form(s) to Web site.
  11. Agencies will be billed annually for maintenance fees.
Note: Some of these cycles occur concurrently.
