The electronic forms (e-forms) on this page are just a few examples of the functionality and flexibility possible with e-forms. These e-forms show what can be done with good design and a user-friendly interface. All of these e-forms have been enabled with Reader Extensions to save the data entered into them. They have also been designed to dynamically change according to a user's selection, and to be submitted electronically.
Change of address (pdf) – This is a simple e-form redesigned from a paper-based form that captures a change of address for both driver license and voter information. These forms are presented in both their English and Spanish versions. There is a charge to create the Spanish (pdf) version, but no licensing and ongoing maintenance charges.

Form, Publication and Web Site Request (pdf) – This is a complex e-form used to request a document. This dynamic e-form shrinks, grows, and sets required fields based on user selections.

PEBB Employee Benefits Enrollment/Change (pdf) – This is a complex e-form for designating a change to an employee's benefits choices. The e-form dynamically adds or removes sections, validates the information entered, and provides feedback to the user. Note how it also tracks pagination, adding appropriate headers and footers as the form grows.

(FFSR) Forms Factory Service Request (pdf) – The FFSR is a form used by Forms Factory to capture requests for Forms Factory services. Both e-form implementation costs and annual e-form costs are calculated to provide an estimate of the total cost of the e-form.

Public Record Inspection/Copy Request and Receipt (pdf) – This is another complex e-form to request public records. This e-form contains several dynamic features, including adding or removing sections, text fields that grow to accommodate large amounts of text, and a 2-D barcode that automatically updates when information is added to the form.
