Enrollment Corrections
If you realize you made an enrollment error, you have the opportunity to correct it within 30 days of recieving your first pay notice. For agency employees, that's Jan. 31; for university emplyees, it's Feb. 28.
Submit the correction form to your agency payroll or university benefits office.
These corrections go into effect the first of the month following receipt of the form. You’re responsible for your portion of premium and claims in your current enrollment until the correction is made.
Qualified Midyear Changes
Some life events qualify for changes in plan choices and coverage levels after the start of the plan year. Examples include getting maried or divorced, having a baby or taking legal responsibility for a child or a child's health care, or moving out of a plan's service area.
Another qualified change happens when an individual you cover - for example a spouse or domestic partner - gains other coverage. This may prompt you to make a midyear change if you have a deduction because your spouse or domestic partner currently waives such other coverage.
To change plans or coverage based on a qualified change, submit the midyear change form to your agency payroll or university benefits office within 30 days of the event. The change goes into effect the first of the month following receipt of the form.
Tobacco-use Status Changes
Your status in the Tobacco-use Program may change during the year. For example, you or your spouse or domestic partner may quit using tobacco, or a medical provider could advise you not to try to quit using tobacco.
These changes allow you to end the related monthly deduction for tobacco use. Submit the tobacco-use midyear change form to your agency payroll or university benefits office within 30 days.
If the form is received by the 15th of the month, it will be processed in that month's payroll and the deduction will end with the start of the following month. If the form is received after the 15th of the month, it will be processed in the next month's payroll, and the deduction will end with the start of the month that follows.
