I signed up for the HEM program and completed the health assessment as scheduled, so why am I slated for a deduction from my pay?
A

If there's an error in the record of your HEM participation, you can take action to correct it. Pease send the following information to PEBB: a completed PEBB Appeal form (click here to download the form) and a copy of confirmation from your plan's website showing that you completed the health assessment as scheduled.

PEBB will use the information to work with your plan to correct any discrepancy within 30 days of receipt of your form and confirmation print out.

Why am I slated for a HEM deduction when my health plan website shows I completed my assessment as scheduled?

If you cover a spouse or domestic partner, the first thing to check is if that individual also completed the assessment as scheduled. If you both completed the assessment as scheduled, please follow the appeal process outlined above.

If you're in a Providence-administered plan:
Log in to your account at https://myprovidence.healthtrioconnect.com/app/index.page. Select “view your health assessment results,” scroll to the bottom of your results page and click “continue.” Print the page.

If you're in a Kaiser Permanente plan:
Sign on at https://healthy.kaiserpermanente.org/health/care/signon and look for “Enrolled” by the yellow sunflower. Print the page.
Q Where do I send the appeal form and copy of confirmation that I completed the HEM health assessment as scheduled?
