Error in Providence Mailing
This information describes an error in mailing by Providence Health Plans and what is being done to correct the error.
 
The week of Dec. 21, 2009, Providence Health Plans prepared a “Get Started” packet for about 42,000 state employees enrolled in PEBB’s Statewide Plan. Providence administers the plan beginning 2010.
 
The packet advised on use of the plan’s myProvidence Web site, through which members may access their claims information. It included a cover letter addressed to the principal subscriber. The letter included names and plan ID numbers of individuals covered by the subscriber’s enrollment.
 
Because of a printing error, approximately 4,500 of the letters had names and plan ID numbers for a different subscriber.
 
The letters did not include personal health information, Social Security numbers, birth dates or other ID information.
 
When Providence became aware of the error, it corrected the mailing and notified PEBB.
 
PEBB and Providence agreed that Providence will:
  • Temporarily take down access to the myProvidence Web site for state employees
  • Send a letter of apology to members who may have received erroneous information, and issue new plan ID numbers for those individuals
  • Send a letter of explanation to all PEBB enrollees in the Statewide Plan with information on use of the myProvidence Web site
 
All PEBB members continue to have access to their benefits and coverage without interruption.
