Medical: Providence Choice
 
You may enroll for Providence Choice if you live or work (at least 50% of the time) in the Portland Metro, Lane, Linn-Benton, Marion-Polk, Deschutes and Curry county. See map below. Click here to view the list of medical home clinics. 
 

 

Medical: Kaiser Permanente
 
You may enroll in Kaiser Permanente if you live or work in a covered zip code (at least 50% of the time). See map below.
 

 

Dental: ODS
 
You can enroll in the ODS Traditional or ODS Preferred no matter where you live or work.
