| Employee Responsibilities |
|
The employee will need to comply with the courts and the Oregon Department of Transportation, Driver and Motor Vehicle Services Branch (DMV) Hardship Permit requirements. If the employee plans to drive on-the-job, either in a state vehicle or private vehicle they must:
- Notify their supervisor, Human Resources Office or agency Director’s designee that their driving privileges have been restricted.
- Learn whether or not their agency will allow on-the-job Hardship Permit driving.
- Obtain private automobile liability insurance as required by the Hardship Permit.
- If the agency will allow on-the-job Hardship Permit driving, agree to abide by:
- The conditions of the Hardship Permit at all times.
- The requirements of their agency´s loss control plan.
- The Vehicle Use and Access Rules, OAR 125-155.
- The state policy that driving must be safe, legal and courteous.
- Obtain from the agency:
- Written permission allowing him or her to drive on-the-job. See DMV form - Requirements for Hardship/Probationary Permit Application (pdf).
- Certificate of Self-Insurance for Restricted Driving issued by the Department of Administrative Services - Risk Management (DAS-RM.) Note: DAS-RM will not deal directly with the employee. We work through the agency Director’s designee.
- Submit all the required forms and certificates to DMV.
- Obtain a restricted operator´s license from DMV.
- Keep agency informed of any changes in status of their driving privileges.
