Health System Transformation - Shared Accountability Aging and People with Disabilities
A system of shared accountability between Coordinated Care Organizations (CCOs) and Oregon's Long-Term Care (LTC) system includes the following goals: 
  • Coordinated and aligned care and services for all individuals getting long-term services and supports.

  • Care and service coordination tailored to unique skills and service environments in long-term care settings.

  • Mechanisms for shared accountability

Strategic Framework

Read the Strategic Framework for ways in which the health care covered by the CCOs can be coordinated and aligned with the long term services and supports provided by DHS. 
 

Memorandum of Understanding Guidance

All CCOs are required to coordinate with the local Area Agency on Aging (AAA) or Aging and People with Disabilities (APD) offices in their region. Review the Memorandum of Understanding Guidance, a resource for CCOs and local offices in establishing this coordination.
