DD 50 Rate Matrix
The DD 50 Rate Matrix identifies the steps in the process of using an individual's assessment information to assign an individual monthly residential budget amount (IBA). The identified monthly allocation also represents the maximum amount the individual's residential provider can bill for supports and services provided.

By June 30, 2011, all 2500 individuals living in DD 50 sites (Group Homes/Apartments) are expected to have completed assessments, assigned budgets and service rates based upon their level of need. An initial 670 assessments were completed by June 30, 2009. Between July 1, 2009 and June 1, 2011 approximately 85 assessments per month for these individuals are being conducted. These assessments are commonly referred to as "Rate Conversion Assessments".
Scheduling and completing the assessment
The Assessment Unit identifies the pool of consumers targeted for assessment within a designated period of time. Each individual's County Services Coordinator will work with an Assessment Specialist and other appropriate parties to schedule and complete the assessment.
Communicating the results and assigning a budget
Generally within 30 days of an assessment: scoring is completed; an individual budget (IBA) is established; and notices of the results are sent.
- The Consumer will receive notice of the results of the ReBAR Assessment, including the Tier that was determined to best represent his/her supports needs in their residential setting, and the effective date that State funds will be available to support him/her at that Tier.
- County Case Manager/Services Coordinator will receive via e-mail the 9-page summary of the consumer's ReBAR assessment and notification of their Tier assignment. The County can provide service rate and assessment results to the consumer, families, guardians and providers, upon request.
- A Residential Provider may also access service rate information for an individual via the eXPRS system (DD electronic payment system).
Identifying DD 50 service rates
As a result of the ReBAR SIS assessment process and new service rates, some service rates (and IBAs) will increase and some will decrease. All rates will be based on the assessed level of support determined for each individual during the standardized ReBAR Assessment process.
The State Contract Administration Unit will notify the County each time a ReBAR Service Element Prior Authorization (SEPA) is prepared with funding available to adjust the service rate for an individual who's ReBAR Assessment has been completed and a support Tier has been determined.

Assessments for mandated caseload individuals entering comprehensive service
As of July 1, 2009, the Assessment Unit is conducting an average of about 50 assessments per month for individuals entering adult comprehensive services from the Mandated Caseload (MCL) due to crisis or transition. The Mandated Caseload includes:
- Adults in "crisis"
- Children in "crisis" including children transferring into DD foster care from Child Welfare foster care
- Children or adults transitioning into adult Comprehensive Services from
- Children's DD funded 24-hour community residential services (Turning 18)
- Adult State Operated (DD 50) Residential Services (SOCP)
- Department of Corrections
- Oregon State Hospital or Psychiatric Security Review Board OR
- Nursing Facilities
Assessment Specialists use the ReBAR SIS Assessment for individuals entering a DD 50 group home. They use the ReBAR Support Needs Assessment Profile (SNAP) for individuals targeted for placement in a foster care setting.
Working with regional and county Crisis staff, the Assessment Unit has established the process for referral and completion of MCL assessments. All communication of information throughout this process is "electronically" secure. The target response time from completion of the assessment to the communication of assessment results is 30 days. Highlights in this process include:
Requesting an assessment
Regional crisis staff works with a County to identify an individual in "crisis" and sends a Request for Mandated Caseload Assessment, DHS Form 744, via secure e-mail to the Assessment Unit. The Form indicates the targeted long term placement setting such as, group home (for which a ReBAR SIS will be conducted) or foster care (for which a ReBAR-SNAP will be conducted). If the target for long term placement is unable to be identified then both assessments are sometimes requested. An Assessment Specialist is assigned to conduct the assessment interview(s).
Scheduling and completing the assessment
Each individual's County Services Coordinator works with an Assessment Specialist, providers and all parties to share assessment orientation information, schedule the assessment, identify the location and necessary accommodations, and remind respondents the day before the assessment. If a consumer is to receive 2 assessments; the ReBAR SIS will usually be done in the morning and the ReBAR SNAP will be done the same afternoon.
- ReBAR SNAP is a portion of the full SNAP assessment. ReBAR SNAP Sections are: Client Demographics, Activities of Daily Living, Medical Needs, Nighttime Needs and Behavior Needs. The Regional Crisis staff completes final sections, if needed.
Communicating the results and assigning a tier for individuals
ReBAR SNAP
The ReBAR portion of the assessment is "locked" and sent to the designated Regional Crisis staff within the 30 day time period. Regional Crisis and County DD Program staff completes the remaining Sections of the SNAP i.e., the 2:1 Rate Review, Day Program Adjustment, Consultants, and Budget Summary sections. The Crisis Region approves a Final SNAP monthly service rate.
ReBAR-SIS Assessment
- The Service Coordinator receives the 11 page SIS Summary report and Tier assignment for the individual. No Service Rate is identified for MCL assessments since the licensed size of the individual's final placement site has not been identified.
- The Regional Crisis Staff will also receive notification of the individual's Tier assignment.
