ReBAR Assessment Tools
The ReBAR SIS Assessment Tool is the adopted name for the Supports Intensity ScaleTM, and the Oregon ReBAR Supplemental Questions with Oregon Fuller Descriptions that have become Section 4 of the SIS Assessment. This Assessment Tool is used to establish the level of support and funding needed by individual consumers.
The ReBAR Support Needs Assessment Profile (SNAP) is a tool developed by the Office of DD Services to determine the level of support needed for children and adults in foster care.
Choosing an Assessment Tool
During 2006, The ReBAR Stakeholders Advisory Group worked to establish Oregon's essential criteria for the selection of an assessment tool. The American Association on Intellectual and Developmental Disabilities (AAIDD) made formal presentations to SPD and stakeholders on the Supports Intensity Scale or SIS. Oregon SPD adopted the groups' recommendation to use the SIS.
Testing the Assessment Tool
During 2007, Oregon conducted a Pilot Test using the SIS. An assessment interview was conducted with a sample of 400 Oregon consumers of group home apartment (DD 50), supported living (DD 51), employment and community inclusion (DD 54) services. Implementing the Assessment Tool
In November 2008, Oregon began using the ReBAR Assessment and developed ReBAR Assessment Orientation Materials targeted for counties, providers, families and consumers.
