Centers for Medicare & Medicaid Services (CMS) approved a $5.5 million grant enabling DMAP to develop and implement a Health Record Bank of Oregon.
Vision
HRBO will increase the amount of health-related information available to clients and caregivers in order to improve the quality of care, health outcomes and client and provider satisfaction while reducing the cost of health care delivery.
 
In October 2007, the federal Centers for Medicare and Medicaid Services (CMS) awarded a $5.5 million Medicaid Transformation Grant that enables DMAP to develop and implement a Health Record Bank of Oregon (HRBO). The grant has an anticipated extension through March 31, 2010. An additional extension of up to one year may be requested. With support from this grant, HRBO will serve Oregon Medicaid clients with the goal of eventually serving other Oregonians. 
 
Vision Statement: Implement a PHR for Oregon Health Plan consumers using an HRB model 
  • Improve health by providing clients with the personal medical information that might not otherwise be available in clinical encounters.
  • Increase client-centered, high quality primary care by supporting coordinated team efforts, even as beneficiaries churn on and off Medicaid.
  • Reduce costs associated with unnecessary and often duplicative diagnostic and other services.
 
Implementation Process: DHS will implement the grant in four phases: 
  • Planning – This phase includes the design of the HRBO, input from key constituencies, investigation of technology options, determination of the HRBO policy framework and the preparation of a procurement process using an RFP Process.
  • Procurement – An RFP will be issued and a vendor selected to build, test, implement and operate the HRBO during the life of the grant and beyond.  Additional responsibilities of the vendor will involve the preparation of recruitment and orientation materials, engagement and recruitment of Medicaid clients to enroll in the Bank, and encouragement of providers to use the program to promote access to more complete information during clinical encounters.
  • Operation – The selected vendor will initiate service and enrollment of account holders, and operate the HRB Oregon on behalf of Medicaid clients. In its recommendations of October 2008, the Oregon Health Fund Board envisioned the HRBO as the vehicle in future years for health information exchange for an increasing number of Oregonians with various sources of sponsorship.
  • Evaluation – The Office of Health Policy and Research will be responsible for evaluating the operation and effectiveness of the HRB Oregon as well as the performance of the grant itself as a tool for transformation.  Lessons learned will enable DHS and its contactors to fine-tune the HRBO for future use.
Organization
Annual Performance Measures

The department’s Division of Medical Assistance Programs (DMAP) and Office of Information Services (OIS) administer the HRBO project.
  • The project operates within DMAP with a staff of three: a project director, a project technology manager and an assistant project coordinator.
  • The project has also engaged the services of consultants with special expertise in health information technology, electronic health information and health record banking to help plan, design and prepare vendor procurement documents.
  • The work of staff and consultants is supported by a small advisory committee comprised of key stakeholders. Oversight for the grant is provided by an executive group within DHS and the governor’s staff.
  • DMAP has and will retain accountability for the operation of the HRBO, but the ultimate governance structure remains to be determined.
