Integration Policy Statement

DHS will systematically organize services in such a way that full integration of mental health, addictions, oral and physical health care is achieved during the next two biennia. Integration means delivering services with the following guiding principles:
Holistic care
services and supports are provided with the whole person as the central focus and careful attention to how symptoms, stressors and disorders interact as well as how interventions are blended to address multiple issues and levels of a person’s life.
Person-driven
services and supports are those chosen to the greatest degree possible by the person/family needing care
Continuity of care
services and supports are provided with as much consistency of provider over time, as in integrated health homes and co-location of services whenever indicated.
Collaborative care
services are provided in collaboration with families, other care providers and natural community supports.
Culturally competent
services are delivered consistent with cultural values and where needed, linguistically appropriate as well.
Prevention and early intervention
services and supports are provided to minimize developmental disruptions for children and adolescents and avoid disability for adults.
Recovery, resilience and wellness
services and supports promote healthy and meaningful lives in the community along with respect for all individuals to achieve their full potential along with the inner capacity to meet life’s challenges.
Evidence-based
services and delivery system are evidence-based wherever possible and consistent with national directions in health care reform.
Measurable outcomes
including measures of quality, access, cost effectiveness and "real life" improvements in health, school, housing, employment and avoidance of premature morbidity and mortality; juvenile/adult justice; homelessness; emergency room use; suicide, etc.
