The Department of Human Services (DHS), Seniors and People with Disabilities, State Operated Community Program (SOCP) is seeking individuals for an exciting career opportunity in SOCP Group Homes. There are thirty-two (32) community-based behavioral and medical homes located in the Willamette Valley area, from Portland to Creswell, Oregon. This recruitment will be used to fill vacancies as they occur. These positions are represented by a union.
This recruitment will be used to establish a list of qualified people. The list will be used to fill openings as they occur. Upon review and determination that you meet the minimum qualifications, your name will be placed on the list of qualified applicants (QLF) that will then be used to select applicants for job interviews.

SOCP provides 24-hour residential and vocational services and support for the highest risk population of Oregon citizens with Development Disabilities. These individuals were either the high-risk population living at Fairview Training Center or represent other individuals with a high potential of risk to the community and themselves. SOCP is dedicated to providing the support necessary to maintain the quality of life, achieve the highest possible level of independence and promote social opportunities, which benefit the individuals and the community. An Individual Support Plan (ISP), which emphasizes what is most important to the individual, is developed annually, updated as needed, and monitored by an assigned county Services Coordinator.

QUALIFICATIONS
Your PD 100 (application form) will be reviewed to verify that you meet the qualifications stated in this section. To receive credit, your application form must clearly show that you have equivalent to:
Six months experience providing direct patient/client care and/or training in a related field of human services (i.e., providing assistance to individuals and/or groups with issues such as substance abuse, aging, mental and physical disabilities or juvenile delinquency); OR
An Associate’s degree in a related human service field.
A drug screening, criminal history check, abuse history and driver's license check will be done on all applicants. If you are a resident of another state, you will need to provide your driver's license record. Any adverse background data may be grounds for immediate disqualification.
You must possess and maintain a current, valid driver’s license and an acceptable driving record.
Your driving record will be considered unacceptable for any of the following offenses. Offenses include but are not limited to: Misdemeanor or felony traffic offense in the last 24 months. This includes reckless driving, driving under the influence of intoxicants, failing to perform the duties of a driver, criminal driving while suspended or revoked, fleeing or attempting to elude a police officer and others; or the equivalent; OR - Felony revocation of driving privileges or a suspension caused by a felony or misdemeanor or the equivalent within the last 24 months; OR – three (3) or more Class B (moving) traffic violations in the last 12 months or the equivalent; OR - Class A violation or the equivalent in the last 12 months.
If hired, either as a permanent or temporary employee, you will be required to successfully complete:
American Heart Association First Aid/CPR certification, requiring you to complete compressions in a kneeling position;
Certification for the Oregon Intervention System (OIS) of restraints, requiring you to get up and down from the floor while maintaining an approved restraint on clients; and
The SOCP system of medical training, requiring you to demonstrate competency in completing doctor’s orders, appropriate passing of medications to clients, appropriate documentation, and appropriate procedures for emergencies.
DUTIES AND RESPONSIBILITIES
The Direct Support Staff provides training and assistance to clients in self-care, routine health care, nutrition and daily activities within the home and community, including support in job placements and other integrated community activities. The Direct Support Staff ensures the home is maintained in a safe, sanitary manner per the Comprehensive 24-hours Residential Services for Children and Adults with Developmental Disabilities OAR Chapter 411, Division 325. Major duties include:
On a daily, or as assigned basis, provides individual assistance, training and support to clients to the degree necessary in the following areas: personal care, communication, medical/behavioral management, and worksite job performance;
Administers and documents accordingly, prescribed medications, special diets including supplements;
Performs interior housekeeping duties such as meal preparation, laundry and cleaning, dusting, emptying garbage, mopping floors, etc.;
Performs exterior duties such as, mowing the lawn, pulling weeds, pruning trees and bushes, etc.;
Maintains continuous observation of resident for behavior/health needs and reports any changes/concerns immediately to the Site Manager or Nurse Manager followed by written documentation on an incident report form, in the medical notes or house log, as appropriate;
Maintains a safe working environment, participates in safety checks and fire drills;
Provides transportation to and from the worksite, school, and community activities, and provides training as appropriate in the use of public transportation system; and
Participates in staff meetings and on-going training opportunities.
WORKING CONDITIONS
Work site is in a home environment, which is staffed 24 hours/day, seven days/week.
Employees in these positions work hours will meet the needs of the home. The hiring supervisor will determine work schedule specifics.
Occasional overtime work may be required to ensure adequate staffing requirements.
May come in contact with potentially argumentative, aggressive and challenging individuals on a daily basis.
You must have and maintain an acceptable valid driver's license, as frequent use of a motor vehicle to transport clients throughout the state to transition program is required.
Individuals in these positions are required the ability to lift up to 75 pounds on an occasional to frequent basis.
Exposed to body fluids, detergents and other cleaning agents on a daily basis.
