These employment opportunities are with Children, Adults and Families (CAF) and Seniors and People with Disabilities (SPD), which are part of the Department of Human Services (DHS). There are continuous openings in various locations throughout the state. There are both full-time and part-time positions. These positions are represented by a union.
This recruitment will be used to establish a list of qualified people and will be used to fill vacancies as they occur. Upon review and determination that you meet the minimum qualifications, your name will be placed on the list of qualified applicants (QLF) that will then be used to select applicants for job interviews. Individuals invited to interview may be randomly selected from the list of qualified applicants.
Please be aware that due to the economic downturn and subsequent state budget short-fall the Governor has ordered mandatory furloughs for employees. This position will be required to take furloughs in the biennium 2009-2011.
QUALIFICATIONS
A Bachelor's degree in a Behavioral Science, Social Science or a closely related field; OR
A Bachelor's degree in any field AND one year of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing); OR
An Associate's degree in a Behavioral Science, Social Science or a closely related field AND two years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing); OR
Three years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing).

DUTIES AND RESPONSIBILITIES
These positions provide case management services to initial applicants and for ongoing participants in Oregon's public assistance programs. The major duties may include:
Work with clients who are in long-term care, including nursing facilities, adult foster care; residential care facilities, assisted living facilities and those who receive care in their own homes.
Determine initial and ongoing financial and service eligibility for public assistance programs.
Perform initial assessments of applicants/recipients to determine financial and service eligibility (CA/PS) for public assistance programs, considering input from family, guardians or other interested parties.
Perform reassessments (CA/PS), at least annually, on all service cases and complete within designated timeframe to avoid interruption of services and payment to providers.
Complete review of financial eligibility at least annually and document as required by policy.
Develop and negotiate case plans with participants to support self- sufficiency; introduce families to other state agencies, community services and resources, partners and employers by making referrals, brokering for and coordinating service needs.
Actively coordinate case management functions with other service providers with open case plan to develop a single case plan for the participant.
Manage the case plan, which includes ongoing assessment of progress and needs, making modifications when appropriate; act as advocate for and empower individuals and their families with other social service entities; issue support service payments.
Provide initial and ongoing eligibility determination for DHS program benefits.
Review, obtain, verify and exchange information. Contact collateral sources to verify information. Examine cases for indications of fraud and refer possible fraud cases to investigators. Determine continued eligibility and adjust benefit amounts based on new or updated information.
Act as an advocate on behalf of clients in interactions with other social service entities.
WORKING CONDITIONS
Travel. You must have a valid driver's license and a good driving record. If not, you must be able to provide an acceptable alternate method of transportation.
May be exposed to emotionally charged individuals with substance abuse histories.
Employees in these positions may be required to work a flexible work schedule as a condition of employment. This is understood to mean that the hours of work may vary from day to day and include evenings and weekends.
