These employment opportunities are with Seniors and People with Disabilities, State Operated Community Program, which is part of the Department of Human Services (DHS). A union represents these positions. This recruitment announcement will be used to establish a list of qualified candidates to fill the current vacancies and may be used to fill future vacancies as they occur.
These positions will be located in community-based behavioral support units located in Salem.
QUALIFICATIONS
Eighteen months of experience providing direct patient/client care or training in a related field of human services (i.e., providing assistance to children or adults or groups with issues such as substance abuse, mental and physical disabilities or juvenile delinquency). OR
An Associate’s degree in a related human services field, AND
You must have and maintain an acceptable Oregon Driver’s License.
A drug screening, criminal history check, abuse history and driver's license check will be done on all applicants. If you are a resident of another state, you will need to provide your driver's license record. Any adverse background data may be grounds for immediate disqualification.
Your driving record will be considered unacceptable for any of the following offenses, including, but not limited to: any misdemeanor or felony traffic offense in the last 24 months. Traffic offenses include, but are not limited to, reckless driving, driving under the influence of intoxicants or other substances, failing to perform the duties of a driver, criminal driving while suspended or revoked, fleeing or attempting to elude a police officer and others, or the equivalent. OR Felony revocation of driving privileges or suspension caused by a felony or misdemeanor, or the equivalent within the last 24 months. OR three (3) or more moving traffic violations within the last twelve (12) months, or the equivalent. OR A Class A violation, or the equivalent, in the last twelve (12) months.

DUTIES AND RESPONSIBILITIES
The Direct Care Staff provide training and/or assistance for the children or adults in, behavioral management, safety, school, personal care, nutrition, housekeeping, health care and other daily activities, both within the unit and the community. Maintain the home/property in a safe and sanitary manner. Major duties include:
There are three (3) major areas that these positions focus on: Behavior Training and Communication and Stabilization/Transition.
Behavior Area
Health & Safety as outlined in each individual’s Behavior Support Plan (BSP) and Individual Support Plan (ISP).
- Follow supervision protocols for each individual both in the home and community settings.
- Follow community transportation protocols and levels of supervision for general community integration, court appearances, school, family visits, medical appointments, counseling and others as needed.
- Follows Oregon Intervention System (OIS) of least to most restrictive strategies.
Treatment Development & Implementation
- Participates and/or provides input in training and treatment development by attending group meetings to develop plans, provides objective feedback to behavior consultant, and assisting with interview/observations as request by the team.
- Implement written ISP and Behavior Support Plans (BSP), which includes proactive and reactive strategies, observes and reports general physical condition.
- Complete Incident Reports as specified by policies and procedures related to behavior and other incidents.
Training and Communication
Training
- Provide training for children or adults living in the home per ISP/BSP.
- Mentors new employees as assigned.
- Communicates clients PFW, and behavior/medical status to co-workers, treatment team and placement providers.
Mandatory Training and Orientation
- Attend and actively participates in job related trainings and implement as appropriate on the job.
- Orient and train prospective caregivers on ISP/BSP.
Stabilization/Transition
- Monitors, assists and motivates children or adults to participate in daily training and treatment needs directed by plan as well as incidental training.
- Monitors and assists with physical cares, personal hygiene, dressing and prosthetic devises, etc.
- Implement transition plan to assist in developing new relationships and familiarize with new placement visits.
- Assist with bridging and building relationships with transition.
- Assist with transition upon exit.
On a daily, or as assigned basis, provides individual assistance, training and support to children or adults to the degree necessary in the following areas: personal care, communication, medical/behavioral management, and worksite job performance;
Administers and documents accordingly, prescribed medications, special diets including supplements;
Performs interior housekeeping duties such as meal preparation, laundry and cleaning, dusting, emptying garbage, mopping floors, etc;
Performs exterior duties such as, mowing the lawn, pulling weeds, pruning trees and bushes, etc;
Maintains continuous observation of resident for behavior/health needs and reports any changes/concerns immediately to the Site Manager or Shift Supervisor followed by written documentation on an incident report form, in the medical notes or house log, as appropriate;
Maintains a safe working environment, participates in safety checks and fire drills;
Provides transportation to and from the worksite, school, and community activities, and provides training as appropriate in the use of public transportation system; and
Participates in staff meetings and on-going training opportunities.
WORKING CONDITIONS
Work site is in a home environment, which is staffed 24 hours/day, seven days/week.
This job requires that the applicant must be able to:
Work hours and shifts in these positions that will meet the needs of the home and will be expected to work any shift, including week-ends and holidays. The hiring supervisor will determine work schedule specifics;
Work occasional overtime required to ensure adequate staffing requirements;
Have and maintain an acceptable valid driver's license, as frequent use of a motor vehicle to transport children or adults throughout the state to transition program is required;
Have the ability to lift up to 75 pounds on a regular basis, including physical restraints;
Work with combative, unpredictable, argumentative, aggressive and developmentally disabled children or adults who may be dangerous to themselves and others on a daily basis;
Withstand exposure to body fluids, detergents and other cleaning agents on a daily basis;
Dress in professional work attire (examples: court, family visits, school, etc) including following house specific dress codes.
