HUMAN SERVICES CASE MANAGER
$15.55 - $22.52 HOURLY


This employment opportunity is with Seniors and People with Disabilities, which is part of the Department of Human Services (DHS). There is one job share (approximately 20 hours per week) opening located in OREGON CITY.  These positions include family health benefits, leave accrual and are represented by a union.   
 
This recruitment announcement will be used to establish a list of qualified candidates to fill the current vacancy and may be used to fill future vacancies as they occur.
 
QUALIFICATIONS
 
A Bachelor's degree in a Behavioral Science, Social Science or a closely related field; OR
 
A Bachelor's degree in any field AND one year of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing); OR
 
An Associate's degree in a Behavioral Science, Social Science or a closely related field AND two years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing); OR
 
Three years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing).
 
Examples of human services related experience include:
 
Assessing clients' needs for services and helping find resources; developing and negotiating case plans; communicating with and assessing the needs of individuals with physical and mental disabilities, or of the elderly who have disease issues; coordinating service delivery systems (e.g., transportation, housing, medical, etc.); acting as an advocate for clients in interactions with other social service entities; providing eligibility determination for social or financial services based on income tests.

DUTIES AND RESPONSIBILITIES
 
The mission of the Seniors and People with Disabilities Office is to provide appropriate social, health care, financial and protective services to all eligible persons in the least restrictive, most cost effective, most appropriate environment.  The local unit determines eligibility and coordinates the delivery of these services within Clackamas County. 
 
This position works primarily with clients who are in long-term care, including nursing facilities, adult foster care; residential care facilities, assisted living facilities and those who receive care in their own homes.  The position determines initial and ongoing financial and service eligibility for public assistance programs.  It acts as liaison and advocate for the client in order to communicate clients' concerns to our agency and communicate agency intent to the client.  It maintains a service ethic with clients, co-workers and the public.  These efforts enable staff to achieve the Agency's mission.  Major duties include:
 
Perform initial assessments of applicants/recipients to determine financial and service eligibility (CA/PS) for public assistance programs, considering input from family, guardians or other interested parties. 
Discuss potential service options with applicants/recipients, considering individual needs, preferences and safety as well as the least costly option of care available. 
 
Complete required documentation and implementation of a service plan within required timeframes, ensuring providers are in place and reimbursed timely for services provided.   
 
Perform reassessments (CA/PS), at least annually, on all service cases and complete within designated timeframe to avoid interruption of services and payment to providers. 
 
Determine if modifications of service plan are needed and complete paperwork related to any changes in facility or provider hours. 
 
Complete review of financial eligibility at least annually and document as required by policy. 
 
WORKING CONDITIONS 
 
Travel. This position requires a valid driver's license and a good driving record. If not, you must be able to provide an acceptable alternate method of transportation.
 
This position is required to make home visits up to two days per week to various areas of Clackamas County and may be exposed to unclean living conditions, communicable diseases, second-hand smoke and pets.
