HUMAN SERVICES CASE MANAGER
(Adult Foster Home Certifier)

$2,695 - $3,903 MONTHLY


This employment opportunity is with Seniors and People with Disabilities, which is part of the Department of Human Services (DHS). There is one full time opening located in KLAMATH FALLS. A union represents this position.
 
This recruitment announcement will be used to establish a list of qualified candidates to fill the current vacancy and may be used to fill future vacancies as they occur.
 
QUALIFICATIONS
 
A Bachelor's degree in a Behavioral Science, Social Science or a closely related field; OR
 
A Bachelor's degree in any field AND one year of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing); OR
 
An Associate's degree in a Behavioral Science, Social Science or a closely related field AND two years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing); OR
 
Three years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing).
 
Examples of human services related experience include:
 
Assessing clients' needs for services and helping find resources; developing and negotiating case plans; communicating with and assessing the needs of individuals with physical and mental disabilities, or of the elderly who have disease issues; coordinating service delivery systems (e.g., transportation, housing, medical, etc.); acting as an advocate for clients in interactions with other social service entities; providing eligibility determination for social or financial services based on income tests.

DUTIES AND RESPONSIBILITIES
 
The mission of the Seniors and People with Disabilities Office is to provide appropriate social, health care, financial and protective services to all eligible persons in the least restrictive, most cost effective, most appropriate environment.  The local unit determines eligibility and coordinates the delivery of these services within Klamath County. 
 
This position is primarily responsible for Adult Foster Home certification and re-licensing but may also include the following:  Determining initial and ongoing eligibility for DHS programs.  The goal of these services are to assure that Seniors and People with Disabilities are able to access and receive benefits for which they are eligible and/or entitle to, in order to maintain their independence and dignity. Duties include:
 
Evaluates information submitted by applicants for new adult foster home license.  This includes, screening applications, assessment and evaluation of provider skills, abilities, knowledge and potential.
 
Inspection of homes for safety and suitability to assure their conformance with rules relating to those facilities.  Approves applications or submits recommendations for denial.
 
Backup to protective service investigators regarding allegations of abuse related to AFH’s and determining if the complaint is substantiated and possible remedies or corrective action.  Investigates complaints of rule violations.
 
Re-licensing of adult foster homes.  Conducts unannounced on-site annual review of program operations for compliance with and adherence to physical facility, record keeping, service delivery, staffing standards for compliance of licensing rules.
 
Participates/ or coordinates development of provider training curriculum.
 
Attends and offers testimony on specific AFH informal conferences and administrative hearings.
 
Determine initial eligibility for Title XIX, Oregon health Plan and Food Stamps.
 
Other duties as assigned.
 
WORKING CONDITIONS 
 
The work schedule for this position is a 40 hour work week.  May require some overtime work before and after scheduled work hours and on weekends.
 
This position requires travel of more than 100 miles per on-way trip.  This position requires a valid driver's license and a good driving record. If not, you must be able to provide an acceptable alternate method of transportation.
 
May come into contact with hostile, angry difficult or mentally disabled people, persons with communicable diseases.
