HUMAN SERVICES CASE MANAGER
(Adult Foster Home Licenser)

$2,816 - $3,903 MONTHLY

This employment opportunity is with Seniors and People with Disabilities, which is part of the Department of Human Services (DHS). There is one full time opening located in GRANTS PASS. A union represents this position.
 
This recruitment announcement will be used to establish a list of qualified candidates to fill the current vacancy and may be used to fill future vacancies as they occur.
 
NOTE:  Please be aware that due to the economic downturn and subsequent state budget short-fall the Governor has ordered mandatory furloughs for employees. This position will be required to take furloughs in the biennium 2009-2011.
 
QUALIFICATIONS
 
A Bachelor's degree in a Behavioral Science, Social Science or a closely related field; OR
 
A Bachelor's degree in any field AND one year of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing); OR
 
An Associate's degree in a Behavioral Science, Social Science or a closely related field AND two years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing); OR
 
Three years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing).
 
Examples of human services related experience include:
 
Assessing clients' needs for services and helping find resources; developing and negotiating case plans; communicating with and assessing the needs of individuals with physical and mental disabilities, or of the elderly who have disease issues; coordinating service delivery systems (e.g., transportation, housing, medical, etc.); acting as an advocate for clients in interactions with other social service entities; providing eligibility determination for social or financial services based on income tests.

DUTIES AND RESPONSIBILITIES
 
The mission of the Seniors and People with Disabilities Office is to provide appropriate social, health care, financial and protective services to all eligible persons in the least restrictive, most cost effective, most appropriate environment.  This position has responsibility for inspecting and licensing existing and new adult foster homes in accordance with state law. Duties include:
 
Inspect all existing and new Adult Foster Homes (AFHs) in accordance with state licensure law, regulations, and agency policy.  Provide forms and assistance to AFH providers to apply for licensure.  Provide guidelines and technical assistance to AFH providers to help them attain compliance with licensure requirements.  Maintain a list of all licensed homes including data about the home, types of clients accepted, types of services offered, and availability of vacancies.  Assist other Case Managers in matching clients to appropriate AFHs.  Reinspect licensed AFHs, noting any deficiencies and continued compliance with regulations.  Recommend continued licensure when no deficiencies exist. Complete and process all required forms and other appropriate documentation to support recommendations to supervisor to approve, deny, grant exceptions, or revoke licenses to operate AFHs. Staff Adult Foster Home licensing issues with management and Protective Services workers. Write sanction notices and civil penalty requests, reprimand letters, and other correspondence relating to licensing enforcement. Prepare documents and testimony for hearings and other legal actions, as directed by SDS management and the Attorney General’s office.
 
Participate in determining appropriateness of care and placement; relate client needs to classification rating of AFHs and provider experience and training.  Insure that the nurse delegation procedure is followed and is properly documented.  Review current doctor’s orders for medication administration and compare them to current medication sheets and prescription bottles to insure that medications are being given as prescribed and that medication administration is properly documented.
 
Interview clients to assess their perception as to the adequacy of their care and their ability to obtain appropriate medical care as necessary.  Follow-up with providers as indicated.  Insure that providers understand their civil and legal liability if clients do not receive appropriate care.  Provide the above services also for private clients in foster home settings (this provides a service not available from the state in any other job description). Refer APS complaints regarding foster homes. Follow up and make referrals as indicated. Recruit and screen foster homes. Prepare and present AFH orientation for new applicants, bi-monthly.
 
Attend branch meetings, supervisory conferences, educational opportunities (including MMIS training), and other meetings as requested. Provide information and referral services to clients, their representatives, and the general public. Utilize ACCESS computer system to input basic client data and perform the screening and intake process.
 
Provide and/or coordinate ongoing training for providers and other professionals as to the requirements and effects of the administrative rules and agency policy.  Assist AFH providers with corrective action when appropriate to do so.  Provide training to other staff on the AFH licensing function.  Maintain a resource book of foster homes to use with clients, families, and the community to promote appropriate foster care placements.
 
Perform other related duties as assigned in conformance with class specifications that clearly fall within management’s right to “direct employees” and “determine methods, means, and personnel by which operations are to be conducted.” Work cooperatively with other staff, other agency representatives, providers, and agency management to maintain professional relationships. May occasionally be assigned to backup staff in another district branch as requested by management.
 
WORKING CONDITIONS 
 
General office environment. Physical exertion such as climbing stairs, walking on uneven terrain, and transporting a laptop. Must travel to homes of clients, hospitals, nursing homes, and other providers as well as other field offices and Salem. May be exposed to unclean living conditions, communicable diseases, second-hand smoke and pets.  May come in contact with irrational, hostile, or angry people, who may become physically assaultive or with a weapon.
 
Drive a variety of cars which may be unfamiliar, to remote places in a variety of terrain and possibly inclement weather.  This position requires a valid driver's license and a good driving record. If not, you must be able to provide an acceptable alternate method of transportation.
