The Department of Human Services (DHS), Seniors and People with Disabilities (SPD), State Operated Community Program (SOCP) is seeking individuals for an exciting career opportunity in state-operated group homes for developmentally disabled adults. There are 32 community-based behavioral and medical homes located in the Willamette Valley area, from Portland to Creswell, Oregon. This recruitment will be used to fill full-time, temporary, part-time, on-call and/or permanent positions as they occur. All permanent positions are represented by a union.
This recruitment will be used to establish a list of qualified people. The list will be used to fill openings as they occur. Upon review and determination that you meet the minimum qualifications, your name will be placed on the list of qualified applicants (QLF) that will then be used to select applicants for job interviews.
ABOUT DHS AND SOCP
DHS is the state’s health and human services agency. The Department’s mission is “helping people to become independent, healthy and safe.” SOCP protects vulnerable Oregonians from abuse and neglect. Our services are delivered in the least restrictive setting and in partnership with communities. SOCP provides 24/7 residential and vocational services and support for the highest risk population of Oregon citizens with Developmental Disabilities. These individuals were either the high-risk population living at Fairview Training Center, or represent other individuals with a high potential of risk to the community and to themselves. SOCP is dedicated to providing the support necessary to maintain the quality of life, achieve the highest possible level of independence, and promote social and vocational opportunities, which benefit the individuals, both within the group home and within the community. An Individual Support Plan (ISP), which emphasizes what is most important to the individual, is developed annually, updated as needed, and monitored by an assigned county Services Coordinator.
DUTIES AND RESPONSIBILITIES
- Monitor and assist residents with physical care, personal hygiene, grooming and dressing activities. Observes, records, and reports illnesses, injuries and behavior changes of residents. Supervises and accompanies residents on shopping, vocational, and other community outings. Encourages and assists residents with appropriate eating habits and table manners.
- Housekeeping – Ensures that residents’ living areas, bedrooms, and bathrooms are neat and clean, and free of potential hazards. Ensures that SOCP rules and protocols are followed concerning sharp implements, locking rooms, and alarm settings.
- Training – Delivers basic individual training to residents, such as toileting, eating, and dressing, following prescribed programs. May assist in instructing or modeling residents in various manual, recreational, and creative activities. Provides input and feedback as necessary through documentation and monthly meetings.
- Behavior Management – Uses positive motivation and/or reinforcement, or other prescribed treatment interventions to effect behavior change. Observes resident behavior. Intervenes, diffuses, and redirects negative behaviors. Records all intervention and redirection of negative behavior.
- Safety Measures – Observes and maintains all safety and security procedures. Answers the needs of residents and assures their general health and safety. Participates in monthly fire drills. Uses appropriate Oregon Intervention System (OIS) restraints as necessary, and per the resident’s Behavior Support Plan (BSP). Safely stores all potentially hazardous materials.
- Record Keeping – Maintains a variety of records pertaining to residents’ care, treatment and training, progress notes, and medical records. Completes Incident Reports as necessary and appropriate.
QUALIFICATIONS
Your PD 100 (Application form) will be reviewed to verify that you meet the qualifications stated in this section. To receive credit, your application form must clearly show that you:
- have interacted with clients with developmental disabilities;
- have accurately observed, reported, and documented behaviors, needs, or incidents;
- have clearly and concisely communicated, verbally and in writing; and
- have a valid driver’s license and satisfactory driving record.
A drug screening, criminal history check, abuse history and driver's license check will be done on all applicants. If you are a resident of another state, you will need to provide your driver's license record. Any adverse background data may be grounds for immediate disqualification.
You must possess and maintain a current, valid driver’s license and an acceptable driving record. Your driving record will be considered unacceptable for any of the following offenses:
- Misdemeanor or felony traffic offense in the last 24 months. This includes reckless driving, driving under the influence of intoxicants, failing to perform the duties of a driver, criminal driving while suspended or revoked, fleeing or attempting to elude a police officer and/or others, or the equivalent.
- Felony revocation of driving privileges, or a suspension caused by a felony or misdemeanor, or the equivalent, within the last 24 months.
- More than three (3) Class B traffic violations in the last 12 months, or the equivalent.
- Class A violation, or the equivalent, within the last 12 months.
If hired, either as a permanent or temporary employee, you will be required to successfully complete:
American Heart Association First Aid/CPR certification, requiring you to complete compressions in a kneeling position; and
Certification for the Oregon Intervention System (OIS) of restraints, requiring you to get up and down from the floor while maintaining an approved restraint on clients.
WORKING CONDITIONS
This job requires that the applicant must:
- work with combative, unpredictable, developmentally disabled adults who may be dangerous to themselves and others;
- work any shift, including week-ends and holidays;
- accompany residents on out-of-town trips; and
- be able to lift 75 pounds on a regular basis, including physical restraints with clients.
