The following materials were created for community partners to share with people who are interested in applying for OHP Standard. You can download and make copies of any of the materials on this page.

We can also send you copies of the poster, brochure and request form while supplies last.

• To order materials, send the following information to: reservation.list.questions@state.or.us
  • Name
  • Organization
  • Mailing address
  • The type of material you are requesting (poster, brochure, request form)
  • The quantity

We will use this information to mail materials to you and we will add you to our list of community partners.

OHP Standard materials revisedBased on feedback we’ve received from our community partners, the department has revised and replaced the informational poster and brochure.On Friday, February 8, we will mail our community partners an initial distribution of the revised materials, which will include:

• A new poster for display in your office, and
• A packet of 50 informational flyers
