How to get on the OHP Standard reservation list

 

When will the list open?

The list officially opened Nov. 1, but due to intense public interest DHS chose to begin accepting requests Oct. 21.

 

What information is needed?

To get on the list, we just need the following information about you and anyone in your household ages 19 through 64 who wants to be on the list:

  • Name
  • Mailing address
  • Date of birth

Printed forms are also available at DHS offices, county health departments and most major hospitals and health clinics. Submit the completed form in any of the following ways:

  • Mail the form to: Oregon Health Plan, PO Box 14520, Salem, OR 97309-5044.
  • Fax the form to: 503-373-7866 or 503-378-6295
  • Drop off the form at any DHS field office (Click here for locations)

Sign up by phone

You can request a reservation by calling 800-699-9075 (phone) or 711 (TTY), Monday through Friday, 7:00 am to 6:00 pm. If you cannot call, you can have anyone call for you. The call will take from 10-20 minutes.
