Re: Meals Made Easy for Diabetes, 2006 – Confidentiality Statement relating to data collected from program evaluation
To Whom This May Concern:
Information collected from 'Meals Made Easy for Diabetes, 2006' evaluation ('Class Log' and 'Participant Information Form' documents) will be used for purposes of internal evaluation analysis for the Department of Human Services, Public Health Division, Oregon Diabetes Program. When data is documented internally, all data will be de-identified, and will only be available to Oregon Diabetes Program staff. All 'Class Log' and 'Participant Information Form' document will be shredded after data is recorded into a password-protected Excel document.
Although DHS evaluation is voluntary, the Oregon Diabetes Program strongly encourages instructors to complete and return the evaluation forms. We will use this information to gain insight into the reach and demographic diversity of the program; these results will also aid us in program quality improvement.
