Ryan White CAREWare - Data Management System:

All service providers receiving Part B funds outside of the TGA (Multnomah, Clackamas, Washington, Yamhill and Columbia counties) are required to use Ryan White CAREWare, the HRSA (Health Resources & Services Administration) recommended data management program, to report services utilization.

3. When have finished filling out the form on your computer - Print it - and FAX it to us. (971-673-0177)
