Annual Performance Measures
Living Well with Chronic Conditions (the Chronic Disease Self-Management Program) is a six-week workshop that provides tools for living a healthy life with chronic health conditions, including diabetes, arthritis, asthma and heart disease. Through weekly sessions, the workshop provides support for continuing normal daily activities and dealing with the emotions that chronic conditions may bring about.
Living Well can help people with chronic conditions make the most of their medical care and communicate more effectively with their healthcare providers. The workshop does not conflict with ongoing programs or medical treatment. It is designed to enhance regular treatment and disease-specific education such as Better Breathers, cardiac rehabilitation, or diabetes instruction. In addition, many people have more than one chronic condition. The program is especially helpful for these people, because it teaches skills that help them manage their health and stay active.
The Oregon Department of Human Services (DHS) supports Living Well/CDSMP workshops in Oregon by providing opportunities for leader training and networking. This is coordinated through Seniors and People with Physical Disabilities and the Health Promotion and Chronic Disease Prevention program.
Living Well with Chronic Conditions workshops are held at various times and locations in Oregon throughout the year.
