Living Well with Chronic Conditions Statewide Forum
The Living Well with Chronic Conditions Statewide Forum is an annual gathering of organizations and individuals who are supporting those living with chronic conditions through Stanford's Chronic Disease Self-Management Program (CDSMP), which includes Tomando Control de su Salud and Positive Self-management Program (PSMP). In many areas of Oregon the CDSMP programs are known as "Living Well with Chronic Conditions."
The fifth annual Living Well Forum will take place on May 6, 2010 at the Best Western Agate Beach Inn, located at 3019 North Coast Highway in Newport.
The Forum will include a keynote presentation by one of the developers of the chronic disease self-management program curriculum from Stanford University. A series of breakout sessions will provide leaders, Master Trainers, program coordinators, and others with an interest in chronic disease self-management with an opportunity to learn more about specific topcs as they relate to Living Well.
The Living Well Network will hold its annual in person meeting on May 7, 2010. Click here for more information on the Living Well Network.

More than 100 participants attended the fourth annual Living Well forum, featuring Dr. Nancy Whitelaw from the National Council on Aging in Washington, DC and a focus on developing partnerships and systems that support chronic disease self-management programs throughout Oregon.

This was the second annual gathering of organizations and individuals who are supporting those living with chronic conditions through Stanford's Chronic Disease Self-Management Program (CDSMP), known in many areas of Oregon as "Living Well with Chronic Conditions." More than 100 participants attended, representing a diverse array of community-based organizations, local and state agencies, clinics, hospitals, health systems, insurance companies, and researchers.

Presentations and Handouts
The first Oregon Living Well Forum held in 2006 was a huge success! Approximately 80 attended this gathering of people using the Living Well with Chronic Conditions program to support people living with chronic health conditions. Topics included: Community Partnerships, Other Self-Management Programs in Oregon, Self-Management in Health Plans and from a from a Provider Perspective.
