Living Well: A Guide to Implementation was developed by the Oregon Living Well Program and Oregon Living Well Network to provide leaders, Master Trainers, program coordinators, and others interested in Living Well with some specific information related to how to implement the program. Each section of the tool includes several resources from Living Well programs across the United States and Oregon, which can be adapted for local use.

| Marketing Materials |

The Oregon Living Well program and the Living Well Marketing and Recruitment workgroup adapted the following statewide marketing materials based on templates developed by the National Council on Aging.
The National Council on Aging developed the templates based on a series of interviews and focus groups with older adults. After conducting market research, NCOA created drafts of marketing and promotional materials for Living Well and tested them with several focus groups of older adults across the country. Using the feedback from these focus groups, NCOA refined the messages, photographs, and graphic images and prepared the templated materials that were provided to state agencies for use in local areas.
The Oregon Living Well program was interested in developing a statewide brand for Living Well to allow for better national recognition of the program, since many states are now using the materials developed by NCOA for their programs.
You may download and print copies of the Oregon Living Well statewide brochure and poster.
You may also download customizable versions of the brochure and poster, so you can insert your own organization name, logos, photos, and so forth. Please see Living Well Marketing Materials: Instructions for Use (pdf) for more information on how to adapt these materials for your organization.

Videos
- Changing Our Lives - "Living Well with Chronic Conditions"
This seven minute video is available in both English and Spanish (Tomando Control de Su Salud) and provides firsthand comments on the impact of self-management workshops in Oregon.
- "Health For a Lifetime"
This five minute promotional video from the National Council on Aging explains the need for chronic disease self-management programs and the benefits of participation. Featuring national spokespeople and participant comments, the video can be used with partner organizations to advocate for use of self-management and evidence-based programs to address healthy aging.
- "En Comunidad"
This three minute Spanish language video includes an interview with Norma Martinez, a Tomando Control de su Salud leader and coordinator of the "Mi Salud en Mis Manos" project at the Farmworker Housing Development Corporation.
Presentations
These powerpoint presentations have been used to promote Living Well programs to community and government organizations, health care providers, Leaders, and program participants. You may find these examples helpful in developing your own. And remember, the Living Well video is a great addition to your presentations!

| Quality Assurance and Fidelity |
|
Stanford's self-management programs result in well-researched health outcomes for participants when programs are implemented as designed (with "fidelity"). Programs are strongly urged to monitor quality and fidelity of community workshops and Leader trainings using the recommended tools below.
