Follow the analysis of an adverse event.

The Patient Safety Commission issues a report crafted by a specially-convened working group. The group looked at the problem of unintentionally retained objects after surgeries in Oregon and made specific recommendations.

Forty ambulatory surgery centers (ASCs) currently are participating in Oregon's adverse event reporting program. Our list includes charter participants as well as newly-joined ASCs.

Oregon's adverse event reporting program for nursing homes currently includes 103 participants, 86 of whom are charter participants
