"On the Move in Oregon" is a Money Follows the Person project funded by the federal Centers for Medicare and Medicaid Services. Oregon's "On the Move" project will demonstrate that long-term institutionalized populations of people with complex medical and long-term care needs can be served in their communities with wrap-around packages of supports and services. Project funding will be available to Oregon from the second quarter 2008 through September 2011. As of July 31, 2009, 83 people have moved back into their homes and communities.
