| The Application Process |

Step 1.
Submit the Department of Corrections application PD291SA to the Recruitment Unit, along with completed Background Information Questionnaire, authorization forms, and PHOTOCOPIES of the following documents:
__ valid drivers' license;
__ social security card;
__ proof of birth; e.g., birth certificate, U.S. Passport, hospital records, immigration papers, etc. ;
__ high school diploma/GED certificate, Diploma's received by non-accredited learning centers will not be excepted. A letter from your high school on school letterhead may be substituted for the diploma; and
__ copy of military DD214/DD215 Discharge Form if you served in the United States Armed Forces. DD214 must be long form, and show the "Type of Separation", and "Character of Service".
Step 2.
ODOC reviews application, Background Information Questionnaire, and required supplements for accuracy and completeness.
NOTE: If the application, and/or Background Information Questionnaire are incomplete, and/or all required supplements are not received, your application packet may be rejected.
Step 3.
Applicant's names are added to the list of qualified correctional officers, and will remain active on the applicant list for twelve months. The different institutions request these lists, as vacancies need to be filled. Your name will only appear on hiring lists matching your geographic availability. Hiring lists are pulled as vacancies occur.
Step 4.
If hired, you will be required to pass a medical exam in accordance with Department of Public Safety Standards Training (DPSST) statutory physical requirements.
Step 5.
Your original application will remain active on the correctional officer list for 2 years. You may request a one time twelve month extension after which you must reapply. Your written request must be received prior to your current expiration date.
