Submit An Address Change
ORS 679.120(4) and ORS 680.075(4) require that all Licensees of the Board advise the Board within 30 days of any change of address. You can do this in a variety of ways: mail, email, fax or submitting the electronic form below. Use of the electronic form is highly encouraged. 

All changes must be in writing, we CANNOT accept changes over the phone.  
 
 
 
 
 
 
Please Note: APO/FPO Address changes cannot be submitted using the electronic address change form. Those who need to change their address to and APO or FPO may send and email directly to lisa.warwick@state.or.us or the print version of the form may be used.
