Self Employment Assistance Program
Instructions for Staff for Enrollment
1. Contact your nearest Worksource Oregon office (Link to offices) and ask if you are eligible for the program. OR Call the Worksource Oregon Training Programs Unit at 1-800-436-6191 and ask the staff to review your Oregon Unemployment claim for eligibility.
 
2. Get a Self Employment Assistance Program application from Worksource Oregon . (form 96-12)
 
3.  Once you have completed and signed your application, fax the form to Training Programs Unit at 503-947-1833 or toll free 1-877-353-7700.  Or you can mail your application to:

Your completed application will be reviewed and scored for feasability by our agency staff and, either approved or rejected. You will be notified of the result.
 
4. Until you have been contacted by the Training Programs Unit telling you that you have been accepted to SEAP, continue claiming your unemployment the same as always and keep conducting an active work search.
 
You will be mailed paper forms on which to claim your unemployment once you have been accepted in to the program.
