Siskiyou Community Health Center offers home visits, medical screening, and weekly support groups to high-risk parents with multiple risk factors in isolated rural areas.
 
“Sarah had everything stacked against her and her newborn daughter, Emily. On top of the stress of being a first-time mom, Sarah also struggled with extreme financial hardships, lack of transportation, a history of addiction and lack of stable housing. She was put in touch with Project Baby Check when her daughter was only a few weeks old and the outreach made all the difference for this family. A Home Visitor began making bi-weekly visits, supporting Sarah’s recovery process, helping her find stable housing, and educating her about Emily’s basic needs and child development.  Over the last three years, Sarah has built a better life for her and her daughter. She recently started a prestigious culinary school, and Emily is an energetic three year-old. Despite all of the hardships, Sarah has worked hard to overcome her obstacles and succeed as a single mother.”
