| Applying for a license |
|
Basic Requirements The basic requirements for licensing include review of your academic education, supervised clinical experience, and passage of a national competency exam. Applications will be reviewed and approved under Laws and Rules in effect at time of application. Application forms are available on the Board's website. In addition to the forms, you must submit a non-refundable license application fee of $172.25, including a fingerprint based criminal background check. Fingerprint based background checks are required for new applicants and every 5 years thereafter. |
