Continued Education Sponsor Forms / Instructions
Please Print Out & Read Instructions First Before Submitting a CE Approval Form.
 
Attention: All CE Sponsor's
 
Our CE Committee Requires All of the Following Items when you ask for CE Approval:
 
1.) Submit One (1) "OBMI Approval Form" For "Each" Continued Education (CE) Request You Are Offering.  Only Electronic Files will be accepted.
 
2.) Submit a "Curriculum Vitae" For "Each" Speaker.
 
3.) Submit a Copy of the "Printed CE Program" being offer.
 
4.) Submit a Sample Copy of the "Certificate of Attendance" that you will be issuing.
 
 
Within 30-days after a CE Offering is given the following Two (2) Additional Items must also be mailed or emailed into the Oregon Board of Medical Imaging.
 
1.) A "Sign-In" List of Attendee's.
 
2.) Class Education Evaluation Form.
