Oregon Formulary & Jurisprudence Exams

The next State examinations in Formulary and Jurisprudence will be held February 10, 2012.

Location: Portland State University
1825 SW Broadway, Room 327
Smith Memorial Student Union
Smith Ballroom, Third Floor
Portland, OR 97201

Time: Formulary Exam - 1:00pm - 2:30pm
Jurisprudence Exam - 3:00pm - 4:00pm

Application Deadline: January 27, 2012

Fees (must accompany application and fingerprint card):
$150 Exam application
$ 50 Background check
Make check or money order to Oregon Board of Naturopathic Medicine.

You may download application materials below or contact this office to have an examination packet mailed to you.

The board must have these three items before you can sit for the exams:

1. Official Naturopathic School Transcripts
2. Diploma from Naturopathic school (can be a copy from your original)
3. NPLEX Scores (for all exams previously taken)

As well as these items for licensure:

1. Official Undergraduate Transcripts*
2. Fingerprint card/passing of background check*
3. Verification from all states/jursidictions in which you have been licensed (current, expired, or otherwise)

* It's a good idea to request your undergraduate transcripts early and to submit your fingerprint card as soon as your application has been submitted

** Please note that the Formulary and Jurisprudence State Exams are in addition to and separate from the NPLEX Basic Science (Part I) and Clinical Science (Part II) examinations.

The next NPLEX examinations will be held on February 7-10, 2012.
