Mandatory Reporting Form (Form #7230 - Stock #300457)
Mandatory reports MUST be submitted on a DMV form. Physicians and health care providers must submit reports to DMV on the Mandatory Impairment Referral Form (see an example of the completed form for reference).
Voluntary Reporting Form (Form #6066 - Stock #300230)
Doctors, health care professionals, family members and social service providers may report an unsafe driver under DMV’s voluntary reporting system. These reports may be submitted on agency letterhead or on a Driver Evaluation Request Form.

Ordering Reporting Forms
Reporting forms may be ordered in bulk by submitting a request to the ODOT Storeroom on agency letterhead. Be sure to include the form and stock number in your request. These numbers are located in the lower left and right corners of the form. Requests for forms can be faxed directly to the storeroom at (503) 986-2801.

DMV has created a brochure series that can be used in talking to patients about safe driving, and planning for a time when they may want to consider “retiring” from driving. To view color copies of the brochures, select the appropriate title below:
Black and white copies of the brochures can be ordered in quantities of 50 from the ODOT Storeroom.
Fax a request for brochures on agency letterhead directly to the storeroom at (503) 986-2801. Be sure to include:
- name of the brochure
- form
- stock number(s)
in your request.

An online presentation on DMV's Medically At-Risk Driver Program is now available. This audio and visual presentation is approximately 12 minutes in length.

| Training Video |
|
Use the online order form to request copies of the Medically At-Risk Driver Training from DMV. The presentation is 18 minutes in length and available on VHS or DVD. The VHS or DVD (please specify your preference) will be mailed to you within five business days of your order.
