DMV issues non-driver identification (ID) cards to people who are domiciled in Oregon and do not already have a valid Oregon ID Card or a current valid driver license or instruction permit from Oregon or any other jurisdiction. An “identification card” is issued for identification purposes.
There is no age restriction to obtain an ID card. ID cards are valid until month/day of your birthday, 8 calendar years from the date of issuance.
You will be required to provide your social security number (SSN) issued to you by the Federal Social Security Administration (SSA). DMV will verify the number you provide with SSA. If your SSN does not verify you will have to resolve the issue with SSA before DMV will be able to issue an identification card. After your SSN is verified with SSA you will need to provide one document as proof of identity and date of birth from List A.
If you have never been issued a SSN you will be required to sign a Statement of No Social Security Number certifying that you have never been issued a SSN and provide at least one document as proof of identity and date of birth from List B.
Applicants that refuse to provide their SSN at the time of application or those that have never been issued a SSN and refuse to sign a Statement of No Social Security Number will be denied issuance of an identification card.
Oregon is a Central Issuance state. Once you meet all requirements for an identification card, you will be issued an interim identification card to use until you receive your permanent identification card in the mail.

| Getting an Oregon ID card |

You must apply for an original ID card in person at a DMV office. To apply, you must:

| Renewing an Oregon ID Card |
|
You must renew an ID card in person at a DMV office. To renew, you must:
- Provide you SSN or if you have never been issued a SSN sign a Statement of No Social Security Number form
- Present the required proof of identity and date of birth
- If your address has changed you will be required to present proof of address
- Pay the renewal fee

| Replacing an Oregon ID Card |
|
DMV will issue a replacement ID card if your ID card is lost, stolen, mutilated or destroyed; or you have a name change or address change. You must do so in person at a DMV office. You will be asked to:
- Provide your SSN or if you have never been issued a SSN sign a Statement of No Social Security Number.
- Present proof of identity and date of birth.
- Present acceptable proof of your name change
- If your address has changed you will be required to present proof of address.
- Pay the replacement fee.
Out-of-State?
If you are currently out-of-state and your ID card is lost, stolen, mutilated or expires you will have to apply for a replacement or renewal upon your return to Oregon at a DMV office. Oregon can not issue a valid without photo ID card.
Change of Address?
If you have a change of address you will be required to submit proof of the new address.
