Due to the passage of SB 1080 by the 2008 Oregon Legislature, documentation requirements for issuance, renewal or replacement of a driver license, driver permit or identification card will change, effective July 1, 2008.

Proof of Identity, Date of Birth & Address Requirements
Important Information
Each time you apply for a driver permit, driver license or identification card you are required to provide proof of identity and date of birth.
Documents must be original or certified copies. Certified copies must be certified by the issuing agency. Photocopies and facsimiles of age, identity, or address documents are not acceptable.
Your proof of identity and date of birth document must contain your full name and date of birth. If you are applying under a name different than what is shown on your proof of identity and date of birth, additional documents will be required to prove your name.
All documents submitted must be acceptable to DMV. DMV has discretion to require additional evidence to verify your identity and date of birth.
Applicants for an Original, Renewal or Replacement DL/ID
Applicants for an Oregon driving privilege or identification card are required to provide his or her Social Security Number (SSN) issued by the Federal Social Security Administration (SSA). DMV will verify the SSN provided on the application with SSA.
If your SSN verifies with SSA you will be required to provide at least one document as proof of your identity and date of birth from List A.
If you have never been issued a Social Security Number (SSN) and are applying for a class C non-commercial driver license, permit or identification card you will be required to sign a “Statement of No Social Security Number” form certifying that you have never been issued a SSN and provide at least one document as proof of identity and date of birth from List B.
Federal regulation requires applicants for a Commercial driver license (CDL) or CDL instruction permit to have a verifiable SSN.
Proof of Residence Address
If this is your first time applying for an Oregon driver license, driver permit or identification card you are required to submit proof of your current residence address. If your primary document used to prove identity and date of birth does not contain your current residence address, you must provide one document from the acceptable Proof of Residence Address list.
Your residence address is the address where you physically reside. It is very important that you provide a good address when applying for a driving privilege or identification card. Failing to provide a good address will cause you to not receive your permanent driver license, driver permit or identification card.
If you are renewing or replacing your Oregon driver license, instruction permit or identification card and your address has changed since you were last issued you will be required to provide proof of your current residence address.
NOTE: If you plan to use your renewal reminder, DMV address change sticker or your vehicle registration/title as proof of your current residence address the document must contain your residence address only. If the document issued by DMV contains both a resident address and mailing address it will not be accepted and you will have to bring in another item from the acceptable Proof of Residence Address list.
NAME CHANGE
A name change can be part of an original, renewal or replacement transaction for a driver license, driver permit or identification card.
When applying for a driver license, driver permit or identification card and your name has changed you must present additional documents proving the name change such as, a U.S. government issued marriage certificate, a certificate of domestic partnership issued by Oregon Vital Statistics, a U.S. court issued divorce decree, judgment of dissolution or annulment of marriage or domestic partnership, an adoption decree, a legal name change decree or a court order.

Important Information
Effective July 1, 2008
Every time you visit a DMV field office to conduct business regarding a driver license, driver permit or identification card you are required to bring proof of your legal presence, full legal name, identity, date of birth, Social Security Number (SSN) and residence address.

The following documents are acceptable proof of legal presence, full legal name, identity and date of birth. One document from the list below is required when applying for an original, renewal or replacement driver license, driver permit or identification card.
Your full legal name is the name shown on your legal presence/identity source document. If your current name is different than the name shown on your legal presence/identity source document, you will be required to provide additional documents proving your name change.
If your date of birth is incorrect on your legal presence/identity source document you will be required to have the source document corrected before it will be accepted as proof of your legal presence, full legal name, identity and date of birth.
Acceptable Proof of Legal Presence, Full Legal Name, Identity and Date of Birth
- A U.S. government-issued birth certificate;
- DMV will not accept birth certificates issued by a non-Government agency, such as a hospital birth certificate, birth card, birth registration or a baptismal certificate;
- The birth certificate can not be laminated.
- A U.S. Consular Report of Birth Abroad (FS-240);
- A U.S. Government issued Certification of Birth (DS-1350 or FS-545);
- Request for Verification of Birth (DD372) must include verification from Vital Statistics with appropriate endorsements;
- Valid U.S. Passport or Passport Card, expired no more than 5 years;
- Valid U.S. Territory Passport, expired no more than 5 years;
- Tribal ID card from or affiliated with an Oregon federally recognized tribe; DMV approved;
- Valid foreign passport, not expired:
- Must include Arrival/Departure Record (I-94, CBP I-94A or I-797) with appropriate entry endorsement.
- U.S. Department of Homeland Security documents, not expired:
- Temporary Resident ID Card (I-688);
- Employment Authorization Document (I-688A, I-688B, and I-766);
- Certificate of Citizenship (N560 and N561);
- Resident Alien Card or Permanent Resident Card (I-551);
- Certificate of Naturalization (N550, N570, and N578);
- Permit to Re-enter (I-327);
- Refugee Travel Document (I-571)
All documents must be original or a certified copy from the issuing agency . All documents submitted must be acceptable to DMV. DMV has the discretion to reject or to require additional evidence to verify your legal presence, full legal name, identity and date of birth.

You are required to provide proof of your SSN for an Oregon driving privilege or identification card.
Why is DMV collecting SSNs?
The Federal Welfare Reform Act includes the requirement that each state collect the SSNs of all drivers to assist in the enforcement of child support laws.
Anyone who has been issued an SSN by the United States Social Security Administration will be required to provide it to DMV. [ORS 25.785, 42 USC 405(c)(2)(C) and 666(a)(13), OAR 735-062-0005] DMV will not issue, renew, or replace a driver permit, driver license or identification card unless you provide proof of your SSN. Customers who are not eligible for an SSN will be required to provide proof they are not eligible. Customers who refuse to provide proof of and disclose their SSN, have never been issued an SSN or refuse to provide proof they are not eligible for an SSN will not be issued an Oregon driving privilege or identification card.
DMV will verify the SSN provided with the Social Security Administration (SSA). If the SSN you provide does not match the Social Security Administration record you will be required to resolve the issue prior to DMV issuing a driver license, driver permit or identification card.
Federal regulations require that applicants for a commercial driver license (CDL) or CDL instruction permit must have a verifiable SSN.
What if I don't have a SSN?
If you have never been issued a SSN by the Social Security Administration and are eligible for an SSN, you will be required to obtain a SSN from the Social Security Administration before DMV will issue an Oregon driving privilege or identification card.
If you are not eligible for a SSN, you must provide proof you are ineligible and sign a Statement of Not Eligible for Social Security Number form certifying that you are ineligible for a SSN.
Who will see my SSN?
DMV takes the responsibility to safeguard personal information very seriously. DMV employees receive training on how to safeguard records and are required to follow state and federal privacy laws. Your SSN will not be displayed on your driver license, driver permit or identification card, or released on DMV records. Access to your SSN is allowed only to qualified government agencies.
Acceptable Proof of Social Security Number
- A Social Security Card issued by the Social Security Administration;
- A document filed with or from the IRS or state tax agency;
- An employment record that includes your Social Security Number;
- A military document from any branch of armed forces that includes your Social Security Number.
Acceptable Proof of Ineligibility for a Social Security Number
- Documents issued by the Social Security Administration, the Department of Homeland Security or other federal agencies or federal courts, which demonstrate you are not eligible for a Social Security Number; and
- A signed certification you are ineligible for a Social Security Number.

You must provide at least one document from this list which proves your residence address if:
- This is your first time applying for an Oregon driving privilege or identification card; or
- Your address has changed since the last time you renewed or replaced your current driver license, driver permit or identification card.
Your residence address is the actual address where you physically reside. It is very important that you provide a good address when applying for a driving privilege or identification card. Failing to provide a good address will cause you to not receive your permanent driver license, driver permit or identification card. A mailing address may be used in addition to a residence address, but never in place of.
If you have an out-of-state residence and/or mailing address you will be required to provide proof that you are a resident of or domiciled in Oregon. You will be required to complete a Certification of Oregon Residency or Domicile form and provide acceptable proof. Proof for residency and/or domicile is noted on the back of the form.
Homeless
Individuals that are homeless may use a descriptive address such as "under the west end of the Burnside Bridge". Applicants needing to use a descriptive address must provide a mailing address and proof that he or she is a resident of or domiciled in Oregon.
You will be required to complete a Certification of Oregon Residency or Domicile form and submit acceptable documents at the time of application. Acceptable proofs of residency and/or domicile are noted on the back of the form.
Continuous Traveler
An applicant who travels continuously may use a descriptive address of "continuous traveler" if the applicant is a resident of or domiciled in Oregon but has no physical home in Oregon. For instance, an applicant who has retired from an Oregon employer may present papers indicating he/she sold a home in Oregon and purchased a motor home.
Applicants wanting to use a descriptive address as “continuous traveler” are required to provide a mailing address and prove that he or she is a resident of or domiciled in Oregon by completing a Certification of Oregon Residency or Domicile form and submitting acceptable proof. Acceptable proofs of residency and/or domicile are noted on the back of the form.
Acceptable Proof of Residence
- Marriage certificate/license containing the current residence address;
- Mortgage documents issued within the current or immediate preceding year;
- Oregon DMV address sticker with residence address only, the sticker will not be accepted if it contains a mailing address.
- A statement from a parent, step-parent or legal guardian of an applicant. The parent or guardian must reside at the same residence address, accompany the applicant and present "Proof of Residence Address";
- A statement from the applicants spouse or domestic partner. The spouse or domestic partner must reside at the same address, accompany the applicant and present "Proof of Residence Address";
- A utility statement of account, hook up or work order dated within 60 days of the application;
- Payment booklet;
- Oregon vehicle title or registration documents containing only residence address
- If the document contains a mailing address the document will not be accepted;
- If using a vehicle registration/title you must be the primary owner of the vehicle.
- Oregon voter notification card;
- Manufactured Structure Ownership documents issued only by Oregon Department of Consumer & Business Services;
- Selective Service card;
- Medical or health card;
- Educational institution transcripts or other forms showing enrollment for the current school year;
- Proof of Enrollment form is not acceptable
- Unexpired professional license issued by an agency in the U.S.;
- Form DS2019, Certificate of Eligibility for Exchange Visitor (J-1) status;
- An approved letter from Oregon State Hospital, Veterans Administration Rehabilitation Clinics, Assisted Care Facility/home, Adult Assisted Living Facility/home, homeless shelters, transitional service providers, or group/half way houses certifying the customer's residence address

Mail from:
- Social Security Administration;
- Government agencies (including state, city and county agencies)
- Mail from DMV can only be addressed to the residence address (mail containing a mailing address is not acceptable;
- Mail from the United States Postal Service confirming an address change is not acceptable.
- Utility companies;
- Insurance companies;
- State and Federal Revenue Departments;
- Originators of out-of-state clearance letters; and
- Educational institutions.
NOTE: Mail must include applicants first and last name and cannot be addressed "in care of", "for", or "parent of". Mail addressed with a “forwarding label” or an “address label” affixed to the envelope or contents are not acceptable. Mail must be dated within 60 days of application.
All documents submitted must be acceptable to DMV. DMV has the discretion to reject or to require additional evidence to verify your residence address.
If you provide DMV with an address that is determined to be an address of a “mail service provider”, “false or fictitious” or an address known to be that of a “business”, you may be denied issuance or your privileges or card may be cancelled until you provide DMV with the address at which you physically reside.
