Replacing a Driver License or Permit
If your driving privileges are not currently suspended or revoked in Oregon or another state, DMV will issue a replacement (Non-Commercial Class C) driver license or an instruction permit if any of the following conditions are true:
  • Your license or permit is lost, stolen, mutilated or destroyed;
  • Your residence address changes and you choose to get a new card;
  • You have a name change;
  • You wish to change, add or delete any description, notation, photo or signature on the license or permit;
  • You are exchanging a valid-without photo license for a driver license with a photograph;
  • You are a former Oregon licensee returning from another state and still have a valid expiration on your previous Oregon license;
  • You are replacing a license issued with an error;
  • You are obtaining a license after reinstatement of a suspension; or
  • You are replacing a license confiscated by law enforcement or the courts

How to Apply
Go to a regular or full-service DMV office. There you will be asked to:
  • Provide your social security number;
  • Present proof of identity;
  • Provide Proof of Residence Address, if your address has changed since your last issuance;
  • Answer medical questions: All driver license applicants must answer a series of medical questions when applying for an original, renewal or replacement driver license (OAR 735-062-0000(2)).  If you answer “yes” to one of the medical questions, indicating you have a medical condition that makes you unable to drive safely, you are not eligible for a license.  DMV will not be able to issue you a license until you have established that your medical condition does not affect your ability to drive safely.  To do so, DMV may ask you to provide additional medical information and/or pass DMV tests; and 
  • Pay the replacement fee.
After you meet all requirements, you will be issued an interim card to use until you receive your license in the mail.
 
Out-of-State/Out-of-Country?
If you are temporarily outside of the state and cannot come to an Oregon DMV office, you may call DMV at (503) 945-5000, send an online request or write DMV requesting a Valid Without Photo Driver License.

Replacing an Address Sticker
If your address sticker is mutilated or has become illegible and you still reside at the same address, you can request a replacement sticker at no cost by calling DMV at (503) 945-5000 or by filling out the general DMV Driver-related Question Form and including your request in Step 4.
 
If you are only requesting a replacement address sticker do not complete another "Change of Address Notice."
