What if I’m in the military?
If you are in the military, you may provide a copy of your Leave and Earnings Statement (LES) showing Oregon as your home of record. You must still provide your actual residence address, even if it is in another state.
I have my mail forwarded to me by a mail forwarding service provider. Can I provide DMV with their address?
You may use the address of a mail forwarding service provider as your mailing address. However, you are required to provide DMV your residence address. This address should be where you physically reside.

If you are temporarily stationed outside of Oregon on active military duty and your Oregon Driver License has been lost, stolen, mutilated, destroyed, or is about to expire, you may apply for a Valid-With-Previous-Photo (VWPP) license or permit.

- Provide proof of your Social Security Number (SSN);
- Complete and sign a Driver License/Identification Card Application;
- Complete and sign a Certification of Oregon Residency and Domicile form;
- Provide a copy of your sponsor's most current Leave and Earnings Statement (LES), showing Oregon as your home of record;
- Provide a copy, front and back, of your valid United States Uniformed Services Identification and Privilege Card, not expired;
- Provide a copy, front and back, of your sponsor's valid Military ID card, not expired; and
- Pay the applicable fees for your transaction.
Order
To order a Valid-With-Previous-Photo (VWPP) packet, fill out and submit the driver question form entirely. In Step 4 of the form, request a VWPP packet to be sent to the address provided. You must include a U.S. phone number or an email address where we can contact you.

| Motorcycle Endorsement |

If you are an active duty military person located out of the state, or their spouse or dependent, you may add a motorcycle endorsement to your valid Oregon License while out-of-state. You will be required to provide a copy of your (or your sponsor's) current Leave and Earnings Statement (LES) that shows Oregon as your home of record.
Important: If you are under 21 years of age you are required to submit a TEAM OREGON Basic Rider Training (BRT) course completion card dated within 2 years of application. There are no other testing options. This card allows DMV to waive the Oregon knowledge and skills tests.
Qualify/Apply
To apply for a motorcycle endorsement, you must:
- Pass Oregon's motorcycle knowledge test or be able to provide other acceptable documentation, as noted in the testing requirements below;
- Pass the Oregon motorcycle skills test or be able to provide other acceptable documentation, as noted in the testing requirements below;
- If under 21 years of age, submit a copy of your TEAM OREGON BRT course completion card (front and back), dated within 2 years of the application;
- Complete a motorcycle endorsement valid-with-previous-photo (VWPP) application packet; and
- Mail the completed application, all supporting documentation, and correct fee (check or money order) to Oregon DMV.
Testing Requirements - 21 Years of Age and Older
For the Oregon motorcycle knowledge test, any of the following are acceptable:
- DMV will work with your base's Education Center to administer Oregon's test, call (503) 945-5114 to provide a contact name and phone number for the Education Center;
- An Oregon motorcycle instruction permit, not expired more than 1 year;
- A passing score on DMV's system for the Oregon motorcycle knowledge test, taken within the last 2 years; or
- A copy of your TEAM OREGON BRT course completion card (front and back) dated within 2 years of application.
For the Oregon Motorcycle skills test, any of the following are acceptable:
- A copy of your Motorcycle Safety Foundation (MSF) Basic Rider Course (BRC) completion card (front and back), dated within 2 years of application;
- A copy of your TEAM OREGON BRT course completion card (front and back), dated within 2 years of the application;
- A copy of your TEAM OREGON Intermediate Rider Training course completion card (front and back), dated within 2 years of the application; or
- A passing score on DMV's system for a motorcycle skills test, taken at Oregon DMV within the last 2 years.
For the vision screening, either of the following are acceptable:
- A Certificate of Vision, Oregon DMV Form 24, indicating your vision meets Oregon's eyesight standards; or
- A passing vision test on DMV's system, recorded within the last 2 years.

To order a Motorcycle Endorsement Valid-With-Previous-Photo (VWPP) application packet, fill out and submit the driver question form entirely. In Step 4 of the form, request a Motorcycle VWPP packet to be sent to the address you provide. You must include a U.S. phone number or an email address where we can contact you.

| Name Change |

DMV will issue a replacement driver license, instruction permit or identification (ID) card if you have a name change. However, you will need to visit a DMV office in person, and you will be asked to:
NOTE: Changing your name on your Oregon driver license, permit, or ID card does not automatically change your name on your Oregon vehicle record, title or registration. You must apply for a name change for your Oregon title within 30 days of the name change.
