Reporting a Problem Driver to DMV
At-Risk Drivers
Family members, friends, law enforcement and social service providers may voluntarily report an unsafe driver under DMV's non-mandatory reporting system.  The report may be based on medical condition or observed driving behaviors.
 
The Driver Evaluation Request form (#6066) can be downloaded from DMV's Web site or obtained from a local field office.  DMV will not accept anonymous reports.  You can request that your name be kept confidential; however, the form requires your name, address and an original signature.
 
When filling out the form, be as thorough as possible.  Describe any medical conditions that interfere with driving, as well as any unsafe driving behaviors that you have observed.  If possible, recount how driving ability has declined over time or as a medical condition has progressed.

 
DMV will review the information and may send a letter to the driver asking for a re-examination of their driving skills.

Violent or Intoxicated Drivers
To report drivers who are intoxicated or are exhibiting signs of "road rage," contact your local law enforcement agency by calling 911.
 
If you suspect the person is driving under the influence then you can call (800) 24DRUNK (or (800) 243-7865) if outside the city limits or 911 if in the city limits.
