Fillable Forms (PDF)
DMV offers selected forms in a "fillable" format (PDF). You may fill out these forms on your computer, print them and send them to DMV. These forms require a recent version of Acrobat Reader to view and/or print.

Please note that it is not possible to electronically submit these forms to DMV -- you must print it out after typing in the information. If the form requires a signature, DMV must receive the form with an original signature.

Please print out and bring with you (or mail) ALL pages of the form. If you go to a DMV Office, this will become your receipt or temporary registration.

All DMV fillable forms have a CLEAR FORM button.

How to Use a Fillable Form
  • To enter data, mouse-click in a text box and a blinking cursor will appear.
  • Press Tab to go to the next field.
  • Press Shift+Tab to return to previous field.
  • To insert a checkmark into a checkbox, use the spacebar or mouse-click in the box.
  • Before printing, tab out of the field in which data was last entered.
  • To print, click on File and select Print. Make sure the checkbox "Print as Image" is checked.
If you experience problems with these forms, non-fillable PDF versions of the forms are available in the Forms section of OregonDMV.com.
