General Information
An Oregon DMV Certified Court Print with CDL Medical Examiner’s Certificate information (MQ) driving record is a “CDLIS MVR” as defined in 49 CFR 384.105 and is a required record in accordance with 49 CFR 391.51 (b) (7) (ii). This is a 10-year driving record that includes both employment and non-employment entries as well as CDL Medical Examiner’s Certificate information. This record does not include drug test result information.

How do I order a Certified Court Print with CDL Medical Examiner's Certificate information?
  • If you do not have a Record Inquiry Account and are requesting a Certified Court Print with CDL Medical Examiner’s Certificate information for an employee, you must use a Request for Information (Form 735-7122). Include any required proof of qualification for personal information as indicated on the Request for Information Form.  Include a check or money order for $3 payable to Oregon DMV.
  • If you have a Record Inquiry Account, you may request this record using the Request for Driving Record (Form 735-48). Your account will be billed $3.

What is the Automated Reporting Service (ARS)?
The Automated Reporting Service (ARS) is a service provided to employers with drivers under their employment, such as trucking companies, delivery services, etc.  This service produces and sends the employer a court print driving record when a conviction, accident or suspension is posted to one of their employee's records.  There is no fee to add or delete an individual to the service if the requester uses DMV's online ARS option; there is a $2 fee for each individual DMV adds or deletes manually.  There is a $3 fee for each printed record produced.  ARS is only available to customers who have a valid Record Inquiry Account. For more information send an email to: ARS@odot.state.or.us.
