For instructions on how to complete the Design Exception Request Form use the link below.
The Design Exception Request must have four signatures:
- Engineer of Record
- ODOT Program Manager, Area Manager, District Manager, Bridge Delivery Unit, Private Public Partnerships, or Local Government
- ODOT Region Tech Center Manager or Region Roadway Manager
- State Roadway Engineer
To submit for State Roadway Engineer approval, the Region Roadway Manager will send the signed original Design Exception Request to:
