|
Please fill out a separate Scholarship Application for each class for which you are requesting a scholarship. If you are applying for a scholarship, please use the same email address you used in your Registration Form. This will prevent repeating your contact information.
Scholarship applications will be evaluated for each class after registration closes. PTD will calculate the scholarship award. The scholarship will support the ESTIMATED costs of your attendance at each event up to the percentage allowed. Please fill out the form completely.
You will be notified of the amount of your Scholarship Award via email. After attending the training, you must submit a Reimbursement Form to receive reimbursement.
